This completed form, along with the test, must be sent early enough to ensure that all materials are received at least three days prior to the test date.
The testing center can only proctor make-up exams. Course exams should be given by the instructor during regular class time. Submission of this form does not guarantee that the time and date have been confirmed. The testing center will contact you regarding the availability of proctors and room assignments.
** Must be a minimum of one full day. Any test(s) not taken by the last day indicated will be returned to the instructor. The location, date, time the test began, and time the test was completed will be indicated on the test by the testing staff.
