Forms Processing

Advisors assist students in filling out and filing a wide array of forms and documents. The table below lists various forms or documents you may use at some point in your college career and why you would need to meet with an advisor about them.

FAFSA (Federal Financial Aid Application) (this can also be done online) No Anyone applying for Federal Financial Aid (Pell Grant). Applicants must also complete the NPC Financial Aid Form and have a Degree Audit Notification on file.
NPC Financial Aid Form No Anyone applying for Federal Financial Aid (Pell Grant)
Degree Audit Notification No All students who wish to pursue a degree or certificate program with NPC
Registration Form Yes All students taking courses at NPC, except those taking only Special Interest classes (099s) and high school students in NAVIT or Dual Enrollment
Registration Form No Students taking only Special Interest classes (099s)
Course Change Form Yes Anyone who would like to add or drop a class or switch to a different section of the same class
Out-of-County Affidavit Yes Any resident of Apache County who is taking classes at a NPC location in Navajo County
Request to Enroll a Child under 14 Maybe (instructor can also sign) Any student age 14 and under who would like to take a course at NPC. Must be approved by the Vice President for Instruction & Student Services
Domicile Affidavit No Any student who has resided in Arizona for less than one year but wishes to attempt a residency claim
New Mexico Resident Tuition Waiver No Any student who is a resident of New Mexico and wishes to apply for one of the limited number of non-resident tuition waivers
Request for a Verification of Admission No Any student who requires official verification of admission to NPC. This form is processed by the Admissions Office.
Application for Admission No All students who wish to take courses at NPC
Petition to Enroll in a Course Taught by a Relative No Any student wishing to take a course taught by a member of the immediate family
Staff Fee Waiver No Any NPC employee who wishes to take a class and qualifies for a fee waiver
Information Release Form No Any student who wishes to grant a third party access to some or all of the records. The form will be kept on file in the student's advising folder
Official Transcript Request No Any student wishing to receive an official transcript or have a transcript mailed to a third party
Request for Evaluation of Credit Yes Any student wishing to have credits from another institution evaluated for possible NPC credit. The student must also have an official transcript sent directly to the NPC Registrar's Office.
Request for Evaluation of Alternative Credit Yes Any student wishing to have work evaluated that is not from an accredited college or university.
